Who we are
Form Digitizer is built and run by Prometheus Labs, a small software company in Kitchener, Ontario. We build focused tools that solve one specific problem well. This one exists because paper forms still run large parts of the working world, and retyping them is the least valuable work anyone does all week.
What it does
Photograph a blank form and get a fillable PDF back, free. Photograph a stack of filled-in copies and get a clean spreadsheet: one row per form, one column per field, with the handwriting read for you and laid out for review. It runs in the browser, so there is no app store, no scanner, and nothing to install.
How the AI reading works
Every page is read by two independent AI passes. Fields where both reads agree are pre-accepted; fields where they disagree, or where the writing is genuinely unclear, are flagged for your review. A crossed-out value replaced with a correction gives you the correction, not both. "N/A" is recorded as N/A, which is different from a field someone left blank. Extracted values are AI-generated until you accept them: you always see and approve the data before it leaves the review screen. AI reading is good, but it is not perfect, so the review step is there to catch mistakes before they reach your spreadsheet.
What happens to your documents
Forms carry other people's information, so the tool holds as little as possible for as short a time as possible. The free converter stores nothing: images are processed in memory and discarded. Signed-in uploads auto-delete after your chosen retention period (default 90 days), and deleting a batch deletes it immediately. Nothing you upload is used to train AI models, and there are no advertising trackers. The full detail is in the privacy policy.
Pricing
Every account starts with 10 free pages. After that you buy pages once and they never expire: $9 for 50, $19 for 150, $49 for 500. No subscription, no per-user seats. Payments run through Stripe; we never see card numbers.

Support goes to formdigitizer@prometheuslabs.ca, and every message gets read and answered. If anything on this site is wrong or out of date, email us and we will correct it.
